The Histopathological Study of Chronic Maxillary Sinusitis


Abstract
The chronic paranasal sinusitis is the one of the most frequent disease in the field of otolarynology and still there are many unresolved problems regarding the etiology, pathophysiology and treatment.
The inflammatory process of sinus mucosa are supposed to be influenced by many factors such as location, age, sex and duration of the disease.
The author studied the mucosal pathology and thickness of chronic maxillary sinusitis considering about in these factors.
The specimens were taken from 48 cases (33 patients) during Caldwell-Luc operation.
The obtained results are as follows:.
1. It was the glandular type which was the most frequently seen among pathologic types in chronic maxillary sinusitis.
2. The thickness of mucosa was greatest at the ostium area among the 6 different locations of sinus mucosa.
3. It was the hypertrophic type which was the thickest among pathologic types in chronic maxillary sinusitis.
4. In each case, there was about 95.8% of difference in pathologic types on the 6 different locations of sinus mucosa.
5. In bilateral case, there was about 32.2% of sameness in pathologic types on the corresponding locations between right and left sinus mucosa.
6. There is no difference in thickness between right and left sinus mucosa in bilateral cases.
7. The atrophic type was significantly increased with age contrary to the glandular type which was decreased by aging.
8. The glandular type was more frequently seen in male and the atrophic type was more in female.
9. It seemed that the increase of the thickness in diseased mucosa was not always proportion to the duration of the disease.
From above results, it was concluded that the sinus mucosal changes were diffrent by portion even in one sinus, and the mucosal pathology seemed to be variable more of less by location of sinus mucosa, and also influenced by age, sex and duration of the disease.
Therefore, the histopathological study of chronic maxillary sinusitis should be discussed with considering some factors which are sex, age, duration of the disease, location of sinus mucosa and etc.
